Your information

The Data Protection Act 1998 enables you to request a copy of the information we hold about you. Therapy

When you, or you and your child, attend for your first therapy session you will be asked to provide information about you and/or your child.  This is information is used to provide the service to you e.g. writing to you with a final therapy report and DVD, knowing your medical history to tailor your treatment.

You will be asked to sign a consent form describing the information we hold about you and the ways in which it might be used. You can choose what is right for you.

We store your personal information up until you reach the age of 25 or for eight years after the date of the last entry and then we will contact you to ask if you would like us to destroy or continue to use it for research, invite you back to the centre and to tell you about new services and information about Bobath Scotland. 

Everyone using Bobath Scotland’s services will have a personal clinical file. This information is used for clinical and research purposes;

  • The information is stored on our secure server and in a paper file, stored in a locked cabinet.
  • We will not share your personal details with anyone else without your permission.

We will ask you if you are happy for your information to be used to send reports or to contact your local team, such as your physiotherapist or a school teacher.

If we are informed that a service user has died, we follow British Medical Association guidance and store their personal records for three years.

Storing information in a database for audit and research purposes

What: We are responsible for ensuring we keep up to date records of any input that we have with you including assessments and outcomes of therapy, together with the information provided by you and professionals involved with you.  

Key pieces of information are collated for every child or adult we see.  This information is stored in a database, enabling staff to quickly access information as a basis for;

  • Service reviews. For example; checking the numbers of children who have had a certain type of assessment used and their results.
  • Identifying suitable people for any research projects we may undertake.

 

Are there any benefits or risks of being included in this database? 

There are no direct benefits to you and/or your child although any research we undertake will help us better understand the role and benefits of Bobath therapy in the management of cerebral palsy. This helps us to provide training improve Bobath Scotland’s services.  Choosing not to participate in this database will not affect the treatment of your child.

Who will use this information? 

Information about your child will not be shared with anyone without your consent or unless required by law.  The database is password protected and can only be accessed by Bobath Scotland employees. Occasionally anonymised information may be checked by others such as a research project supervisor or representative from a research ethics board if you have given your permission for your information to be used for research.  Any publications produced from the audits or research undertaken would be anonymised.

The taking and use of photos and recordings

During therapy it is useful to take audio/video/photographic recordings. This can help us to analyse movement for example, as a basis for making treatment decisions and allows us to provide visual information within reports.  Please note that you are under no obligation to provide consent for us to take photos/recordings – and your treatment will not be impacted as a result should you wish to decline.

Here are other ways that photographs and recordings are used;

For therapy: Where time permits we try to provide an edited film of the most helpful parts of the block.  A copy of the filmis kept in your therapy file at Bobath Scotland.

For internal training of Bobath Scotland staff: There is a regular programme of internal training – during some sessions we use case studies as a means of analysing our work and generating new ideas for therapy. 

For external training: As a specialist centre, Bobath Scotland is often involved in external training of support and professional staff who work with people who have Cerebral Palsy.  This can include staff from heath, social care and education. 

For publications and promotional materials:  From time to time we are involved in activities such as producing articles about the service provided at Bobath Scotland for various publications.  Occasionally we produce posters regarding our work and projects we have undertaken.  It is helpful for us to know if you are happy for photos of you to be used.  We would contact you to let you know in advance if we intended using a picture in this way.


Fundraising

Donations to Bobath Scotland: When you make a donation or attend event, we will create a record of the financial transaction, as well as storing your name, address and post code together with your Gift Aid declaration. If you make a gift online, we will also ask for your email address.

This is the minimum amount of information we need to ensure that we have a suitable audit trail for the donation and for HMRC.

Keeping in touch

When you make a donation to Bobath Scotland, we will write to your to say thank you, both post or by email. You can unsubscribe from any future emails by following the link in the thank you email or by contacting us on 0141 352 5000, 10 High Craighall Road, Glasgow G4 9UD or info@boabth.scot.

You can tell us if you’re happy to receive information about fundraising and events by phone, post or email. We don’t use SMS or external agencies.

In addition to this, we promise never to cold call you or share your data and personal details with other organisations.

Gift Aid

In order to maximise your giving and claim a further 25% we must have your name, house number and post code. We also need to keep a record of your Gift Aid Declaration, whether it is made in writing, through ticking a box or telling someone who works for Bobath Scotland.

Bobath Scotland simply would not exist without fundraising and, more importantly, its donors and fundraisers. We try always to adhere to best practice and act transparently.
